This systematic qualitative review aimed to explore the therapeutic effects of Mindfulness-Based Cognitive Therapy on the reduction of psychosomatic symptoms in patients with chronic physical illnesses. Psychosomatic symptoms, including pain, fatigue, gastrointestinal discomfort, and tension, are commonly reported by individuals living with long-term conditions and are often inadequately addressed by conventional biomedical treatments. Guided by a constructivist paradigm, this study synthesized findings from ۲۱ peer-reviewed qualitative and mixed-methods studies published between ۲۰۰۰ and ۲۰۲۴. Thematic synthesis was used to analyze patient-reported experiences of MBCT. The review identified several recurring themes: enhanced body awareness, reduced symptom preoccupation, improved emotional regulation, and the adoption of adaptive coping strategies. Participants reported a shift in their relationship with bodily sensations, moving from fear and avoidance to acceptance and mindful observation. Additionally, many experienced improved interpersonal communication and a renewed sense of agency. Variability in engagement and benefit highlighted the importance of personal, cultural, and contextual factors in influencing treatment outcomes. The findings support MBCT as a meaningful, non-pharmacological intervention for addressing the psychological components of chronic illness and suggest its potential for inclusion in integrative care models. Recommendations include expanding MBCT access, providing training for healthcare professionals, and conducting longitudinal studies to assess sustained impact. This review underscores the relevance of mindfulness-based approaches in enhancing the quality of life for patients with psychosomatic distress.
