Background and Aims: Medication adherence is one of the most significant therapeutic challenges in elderly populations, particularly among individuals with chronic diseases. Non-adherence to medications is associated with multiple negative consequences, such as an increased risk of mortality, frequent hospital admissions, and a decline in quality of life. In recent years, advancements in technology and artificial intelligence (AI) have provided opportunities for innovative solutions to improve medication adherence. The aim of this systematic review is to examine the application of AI in enhancing medication adherence in elderly individuals with chronic diseases. Methods: The present study is a systematic review. A search was conducted in the databases PubMed, Scopus, Web of Science, and Google Scholar using the English keywords "artificial intelligence," "medication adherence," "chronic diseases," and "elderly" for the period from ۲۰۱۵ to ۲۰۲۴. Inclusion criteria were studies focusing on AI and its application in improving medication adherence in elderly patients with chronic diseases. Analysis was performed using qualitative content analysis. Results: The results of the study led to the identification of two main categories: "Identifying Patients at Risk of Non-Adherence," which included four subcategories (clinical data analysis, identifying risk factors, predicting non-adherence, and providing recommendations and interventions), and "Providing Reminders and Personalized Interventions," which included four subcategories (regular reminders, personalized interventions, continuous follow-up, and patient communication). Conclusion: The findings suggest that AI plays a crucial role in improving medication adherence among elderly patients with chronic diseases. Intelligent systems can help enhance medication adherence by identifying patients at risk of non-adherence and providing personalized reminders and interventions. These results indicate that the use of AI can contribute to overall healthcare improvement and reduce the burden of chronic diseases in elderly populations.
