Introduction: Since the diagnosis of osteomyelitis in patients with diabetic foot ulcer is important, in this study we decided to examine the effectiveness and value of procalcitonin in the diagnosis of this condition. Method: During a cross-sectional study, the researcher examined the patients by referring to the Babol Rouhani hospital and clinic. After checking the entry and exit criteria, patients were included in the study. Demographic information including gender, age, marital status, education level and occupation were recorded. Then, they were divided into two categories: diabetic foot ulcer without Osteomyelitis and diabetic foot ulcer with osteomyelitis, and they were followed up for one year during treatment. Also, other information including examinations, laboratory tests and imaging documents were recorded. All information was entered into SPSS V۲۲ software and analysis was done. Results: ۱۲۸ people entered. ۵۶.۳% of participants were male. ۵۷% of people were in the age range of ۶۱-۸۰ years. ۵۷.۸% of people had osteomyelitis. ۱۰.۹% had a history of kidney disease, ۳۹.۸% had a history of heart disease, ۴۵.۳% had a history of hyperlipidemia, ۵۷% had a history of hypertension, and ۳۳.۶% had grade ۳ diabetic foot ulcer. There was a significant relationship between age and blood pressure with osteomyelitis (P value < ۰.۰۵), but there was no significant relationship between sex, kidney disease, heart disease and hyperlipidemia with osteomyelitis. All PCT, WBC, HbA۱C, ESR, CRP, Cr, Cholesterol tests were significantly higher in people with osteomyelitis than in the other group. ESR and CRP and PCT were effective predictors of osteomyelitis. There was also a borderline positive correlation between PCT and ulcer grade and CRP. Also, at ۰/۰۸۵ ng/ml, PCT was ۹۵/۹% sensitivity and ۸۳/۳% specifity. Conclusion: Considering the high sensitivity and specificity of procalcitonin compared to other investigated factors, this factor can be useful in the diagnosis of osteomyelitis caused by diabetic foot ulcer.
