The Relationship between Polycystic Ovary Syndrome and the Occurrence of Recurrent Pregnancy Loss: A Systematic Review

Introduction: Polycystic ovary syndrome (PCOS) is a common endocrine disorder that affects ۶ to ۱۰ percent of women of reproductive age. Pregnancy in women with PCOS is associated with common pregnancy complications such as recurrent pregnancy loss (RPL). In this study, we investigate the relationship between PCOS and RPL. Search Strategy: In systematic review study, the relationship between polycystic ovary syndrome and the occurrence of recurrent pregnancy loss was performed from SCOPUS, PubMed, SID databases and Google scholar search engine with the keywords PCOS and recurrent pregnancy loss and with the help of Mesh. ۹۸ articles were found in the initial search. After removing duplicates, reviewing the titles and abstracts, and selecting the most relevant articles, ۸ articles that were accepted in the years ۲۰۱۹ to ۲۰۲۴ were studied and reviewed. The exclusion criteria were not having the inclusion criteria and not having access to the full version of the article. Results: RPL occurs in approximately ۱.۹% of couples. Recent research has determined that the cause of RPL is very varied. Several factors associated with it have been investigated that affect the risk of pregnancy loss in PCOS. PCOS is a multifactorial disorder that affects the female reproductive system. Pregnant women with PCOS have an increased risk of adverse pregnancy outcomes, independent of subfertility and the use of assisted reproductive technology. An increased risk of gestational diabetes mellitus (GDM), preeclampsia (PE), pregnancy-induced hypertension (PIH), preterm delivery, caesarean section, abortion, neonatal hypoglycemia and perinatal death have been reported in the PCOS population. RPL in PCOS occurs in ۵۰% of all pregnancies. The clinical association between RPL and PCOS is over common, however, in approximately ۵۰% of cases, pathophysiology remains unknown. Conclusion and Discussion: According to studies, women with PCOS should be aware of the increased risks of pregnancy, including early pregnancy loss. Healthy preconception therapy is strongly recommended for screening for the disorder with lifestyle interventions or specific treatments.
