Introduction: The Objective Structured Clinical Examination (OSCE) is one of the common methods for evaluating the clinical skills of nursing students. However, this exam often causes severe anxiety in students, which can negatively impact their performance. Conducting MOCK OSCE exams before the final exam can help reduce this anxiety. The aim of this study was to investigate the effect of MOCK OSCE on test anxiety and improving the performance of first-semester nursing students in the OSCE.Methods: In this study, ۶۵ first-semester nursing students in the academic years ۱۴۰۲-۱۴۰۳ were randomly divided into intervention and control groups. The intervention group underwent a MOCK OSCE program, which included exam scenarios, feedback, and practical exercises. The control group received the regular course program. Students' test anxiety before and after the intervention was measured using the Sarason Test Anxiety Questionnaire, and their performance in the final OSCE exam was evaluated using standard checklists.Results: The results showed that test anxiety in the intervention group significantly decreased after the MOCK OSCE (p<۰.۰۰۱, R=-۰.۹۱). Additionally, the performance scores of the intervention group in the final OSCE exam were higher than the control group (R=۰.۵۹, p<۰.۰۵). However, the test anxiety score did not have a significant effect on the students' performance in this study (p<۰.۰۵, R=-۰.۲۳).Conclusion: Conducting MOCK OSCE exams can help reduce test anxiety and improve the performance of nursing students in the final OSCE exam. The use of this method in the evaluation program for nursing students is recommended
