Background and purpose: Polycystic ovary syndrome (PCOS) is a common endocrine disease that causes problems for women getting pregnant. In modern society, people pay more attention to the female factor, but it is not clear whether sperm quality is another factor that affects the pregnancy outcomes of patients with PCOS.Method: The effect of sperm DNA fragmentation (SDF) on oocyte fertilization, embryo development and pregnancy outcomes in patients with PCOS who underwent in vitro fertilization (IVF) treatment was investigated. A total of ۱۲۶ PCOS patients and ۱۵۰ control patients were selected for IVF treatment. All female patients were divided into two groups based on the DFI of their husbands' sperm, the DNA fragmentation index (DFI) ≤۱۵% and DFI>۱۵%.Results: There was no difference in baseline clinical characteristics between couples with DFI ≤ ۱۵% or DFI > ۱۵%. For control patients, no difference in IVF outcomes was observed. However, for PCOS patients, there was no difference in conception ۶۰.۴% vs. ۵۹.۹%, clinical pregnancy ۷۸.۴% vs. ۶۶.۷%, but a significant difference in high quality embryos ۲۶.۳% vs. ۱۶.۳%, p = ۰.۰۲۳ and miscarriage ۱۲.۵ % vs. ۱۶.۵%, p=۰.۰۱۲, was observed for couples with sperm DFI > ۱۵% compared to sperm DFI ≤ ۱۵%.Conclusion: For PCOS patients undergoing IVF, fertilized eggs using sperm with higher DFI resulted in the formation of lower quality embryos and increased the miscarriage rate, but had no effect on fertilization, number of embryos, and clinical pregnancy
