چکیده مقاله:
Background: Upper limb amputation due to trauma leads to disturbance in physical and psycho-social aspects which makes it difficult for reintegrate to life. This research aimed designing and evaluating care plan for re integration to life in patients following upper limb amputation. Methods: This study is a several complex Mixed Methods design using Medical Research Council framework. Development phase was done by an integrated review and qualitative research. In qualitative part participants were ۱۳ consist of patients with upper limb amputation, caregivers, nurses, orthopedic surgeons and physiotherapist. Conventional content analysis approach was used. Care plan was finalized by panel of experts then pilot test, was done. Findings: Finalized complex Care plan is developed in six dimensions according to Integrated Care Model: Social needs, Treatment system, Rehabilitation program, Support in self-care, helping in timely and correct decision-making, Teaching patient and family. Conclusion: Reintegration into life care plan can be implemented in a multidisciplinary manner with emphasis on key role of nurse as coordinator, patient center care by involving patients and family facilitate reintegration into life following upper limb amputation.
