Sperm DNA Fragmentation Index(SDFI) has been proposed as a candidate test forthe assessment of sperm integrity in assistedreproductive technologies. It has been assumed thatdamage to the sperm chromatin is associated withdetrimental reproductive outcomes, but the resultsof different studies are controversial. The objectiveof our study was to investigate whether spermDNA fragmentation testing has a prognostic valuefor Intra-Cytoplasmic Sperm Injection (ICSI).Material and Methods: This was aretrospective cohort study conducted at Kamaliinfertility center. SDFI was measured throughspecial kits based on Halo test assay on the freshsemen sample presented on oocyte puncture day incouples undergoing ICSI treatment. Thegeneralized estimating equation (GEE)-model andmultivariable model were used to analyse theprobability of clinical pregnancy and live birth rate(LB), respectively. ROC analysis was carried out todetermine an SDF cut-off.Results: There was an inverse relationshipbetween SDF in the ejaculate of the therapeuticsemen sample and fertility outcomes (p = ۰.۰۲) aswell as LB (p = ۰.۰۴). The ROC analysis proposeda cut-off of ۱۷.۵% as the best compromise betweensensitivity and specificity in the diagnostic SDFIfor live birth; however, the test diagnostics are low,with an AUC of ۰.۵۷۶.Conclusion: Overall, this study strengthens thehypothesis of an inverse relationship between SDFIand LB. Furthermore, SDFI taken together withother clinical characteristics might provide moreinsight into male reproductive potential andpredicting ICSI outcomes. Couples with SDF ≥۱۷.۵% in the therapeutic semen sample reduce thechance of LB.that anticipate your paper as onepart of the entire proceedings, and not as anindependent document.
