Thyroid hormones profile in pediatrics with nephrotic syndrome in comparison to healthy children

Introduction: Nephrotic Syndrome (NS) is one of the most common kidney diseases in pediatrics. It has impact on thyroid hormones as a result of urine loss of hormone-binding proteins such as transthyretin, thyroxine binding globulin (TBG), and albumin. Objectives: This study aimed to compare the thyroid hormones profile between children with definite diagnosis of NS and healthy ones. Patients and Methods: This retrospective case-control study was performed in a pediatric university hospital between Jan ۲۰۱۱ and Dec ۲۰۲۱. Case group consisted of all children with definite diagnosis of nephrotic syndrome who referred to our hospital, and healthy children, who referred to hospital because of routine checkup, enrolled randomly in control group. All variables were compared between case and control groups. Results: The median age were ۵.۹۱±۴.۱۴ years and ۶.۰۷±۳.۷۰ years in control and case group, respectively. TSH and T۴ were significantly higher and lower in case group than control, respectively (P-value > ۰.۰۵). There is no significant difference between case and control group for Free T۴ (۱.۳۰±۱.۹۵ versus ۱.۱۲±۰.۲۴ (ng/dL), P-value: ۰.۵۵۸). Conclusion: Subclinical hypothyroidism is more common in NS children. Actually, it may be due to the albuminuria and loss of TBG in active proteinuria stages.
