In recent years, studying the connection between Health Psychology and Chronic Disease has become more important. Working with chronic patients has always been difficult and challenging and closing the therapeutic alliance with these patients is very important. In this article, we have provided a literature review the Challenges of Health Psychology and therapeutic alliance in working with chronically ill individuals. Is it difficult for health psychologists to create therapeutic alliances with chronic patients? Are there any differences between the therapeutic alliance components in working with mental patients and patients with physical problems? Searching for research history using the following electronic databases including google scholar, PubMed, ProQuest, Ovid, and Science Direct since ۲۰۰۰ Up to ۲۰۲۳ were collected. In this research, it was shown that the therapeutic alliance components in working with chronic patients are the same as mental patients. But the challenge of working with doctors and patients who feel hopeless or unsure remains.
