Introduction: Hypertension is one of the most common complications of pregnancy, which is the main cause of maternal and perinatal disability. Preeclampsia is a disease of pregnancy with severe fatal complications such as diffuse intravascular coagulation, cerebral hemorrhage, liver failure and acute renal failure, which can be prevented with proper control of blood pressure. Due to the high mortality rate and complications of severe preeclampsia worldwide, especially in developing countries, and the inconsistencies regarding the therapeutic effects and side effects of nifedipine and hydralazine, and in order to compare the therapeutic efficacy of these two drugs, the present study The aim was to compare the effect of hydralazine and nifedipine on the reduction of acute hypertension in women with preeclampsia referred to Asalian Hospital in ۲۰۱۸Materials and Methods: In this single-blind clinical trial (RCT) study, all pregnant women with preeclampsia with acute hypertension (BP≥۱۶۰/۱۱۰) who meet the inclusion criteria will be assigned to the study groups after obtaining informed consent. They will be randomly assigned to the study groups in pairs and individually. The subjects were divided into two groups of ۳۰ people, in one group hydralazine injection and in the other group nifedipine was taken orally.Results: The results of the present study showed that in terms of maternal and neonatal outcomes and complications such as maternal headache, maternal hypotension, maternal flushing, abnormal heartbeat, type of delivery, hospitalization status, HELLP syndrome and eclampsia statistically between There was no significant difference between the two groups (P> ۰.۰۵). Also, in the present study, there was no statistically significant difference between the effectiveness of oral hydralazine and nifedipine in lowering blood pressure. But there was a difference between the mean time to reach target blood pressure between the two groups. There was a significant indication of a faster effect of hydralazine than nifedipine on lowering blood pressure.Conclusion: There is no difference between hydralazine and nifedipine in terms of maternal and neonatal outcomes and complications such as maternal headache, hypotension, hot flashes, abnormal heartbeat, type of delivery, neonatal status, HELLP syndrome and eclampsia. Part of oral hydralazine and nifedipine are similar in lowering blood pressure. The time to reach the target blood pressure in hydralazine is shorter and faster than nifedipine.
