Examining the barriers and necessary solutions for using telemedicine in the Covid_۱۹ pandemic in Iran

Introduction and objective :During the covid-۱۹, on the one hand due to the high speed of disease Transmission in the country and on the other hand, the rapid advances in information technology, there is a need to design and apply new health care models to prevent "face to face" communications between clinical professionals and patients. As a result, this review study has been conducted with the aim of investigating the problems and necessary solutions for using telemedicine in the Covid-۱۹ epidemic in Iran.Methods: The present review study using the information collected from the articles published in the last two years by applying the free full text filter is related to the obstacles and necessary solutions for the use of telemedicine in the covid-۱۹ epidemic. Articles related to this study were searched using by mesh keywords through scientific sources such as PubMed, Google Scholar and Scopus. The required information were extracted and the obtained results were analyzed and interpreted.Result: The results of this study showed that telemedicine reduces the speed of infection transmission and avoids the need for patients to visit directly during the epidemic. In a study, insurance and reimbursement (۸۹.۱%), initial infrastructure costs (۸۲.۶%) and lack of technical staff ۸۰.۴%)) are known as the most basic obstacles for the implementation of telemedicine. To overcome these obstacles, solutions Training of employees, provision of necessary costs for technical infrastructure and hiring of technical forces suggested. Doctors as the most important group to implement telemedicine know the fear of mistake diagnosis, privacy of patients and reimbursement an important barrier to implementation telemedicine services, which depends on the training needed to improve medical visits.Conclusion: This study showed that the problems related to the use of telemedicine are divided into eight aspects: legal, behavioral, cultural, clinical, financial, organizational, sub-structural and finally technical structure; and due to the low speed of information transmission, high cost and high volume medical data for transmission, telemedicine capabilities in Iran are seen as very limited and in certain places that by providing internet infrastructure and suitable hardware, Iran can move forward with modern technology.
