Report of a case of suicide with Metronidazole tablets

Introduction : Metronidazole is an antibiotic drug for anaerobic bacteria and an antiparasitic. This drug is used alone or together with other antibiotics to treat pelvic inflammatory disease, endocarditis and bacterial vaginosis. About ۸۰% of oral metronidazole is absorbed. Food delays the rate of absorption but does not affect the amount of absorption. Metronidazole is metabolized to an active metabolite (۲-hydroxymethyl). About ۲۰% is excreted unchanged through urine and about ۶-۱۵% through feces. The half-life of metronidazole in adults with healthy kidneys is ۶-۸ hours. Metronidazole tablets may cause side effects such as dizziness, headache, diarrhea, nausea and vomiting, stomach pain, loss of appetite, constipation and dry mouth.Purpose: The aim of this study was to report a case of suicide with Metronidazole tablets.Patient introduction: A ۳۰-year-old female patient committed suicide with ۶۰ tablets of metronidazole ۲۵۰ mg after a verbal conflict with her husband and went to the emergency room. The patient complained of severe headache in the temples on both sides of her head and experienced severe nausea in the emergency room. Ondansetron ampoule ۴۰ mg was injected into the serum. The patient's vital signs included BP=۱۲۰/۸۰, HR=۸۰, RR=۲۰, and T=۳۶.۸. Stomach lavage was done for the patient. Angi tube was inserted for the patient and while requesting Na, K, PT, PTT, ALT, AST, Alp, BS, Ca, BUN, Cr, CBC, VBG tests and taking ECG of the patient under treatment with ۶۰ grams of charcoal + ۶۰ grams of sorbitol were placed. The results of the patient's tests were as follows: BS=۹۵, BUN=۱۰, Cr=۰.۷, Ca=۹.۸, ALT=۱۵, AST=۲۷, ALP=۲۳۸, PT=۱۲, PTT=۳۲, Na=۱۴۰, K= ۴.۱, Hb=۱۴.۸, Hct=۴۳, PH=۷.۳۴, Pco۲=۳۷.۲, Po۲=۶۹.۴, Hco۳=۱۹.۶. The patient was instructed to go to the emergency room in case of severe nausea, abdominal pain and changes in consciousness to improve his mental condition. After the relevant treatments, the patient was discharged from the hospital with a good general condition and personal satisfaction.Discussion and conclusion: Metronidazole tablets are available under the brand names Flagyl (immediate-release) and Flagyl ER (extended-release). Immediate-release drugs are released into the body immediately. Sustained-release drugs will slowly release in the body over time.
