Comparing Diagnostic Accuracy of the fliD Gene and theglmM Gene in Helicobacter Pylori

Background and Aim : Helicobacter pylori (H. pylori) is one of the most common humanbacterial infections, accounting for the infection of half of the world’s population. The polymerasechain reaction (PCR) has high specificity and sensitivity in diagnosing this bacterial infection. Thepresent study aimed to compare the sensitivity and specificity of the fliD gene and the most widelyused glmM gene in the PCR technique.Methods : This cross-sectional study enrolled patients with indications for upper endoscopy fromApril ۲۰۱۹ to April ۲۰۲۰ at Qom, Iran. Determination of H. pylori was done on endoscopicbiopsies using our described gold standard which combined of Rapid Urease Test (RUT) andhistopathological techniques, simultaneously. Then, the DNA extraction and PCR assay wereconducted for our proposed gene (fliD) and the most widely used glmM gene. At the end, wecompared the sensitivity and specificity of these ۲ genes in detection of H. pylori.Results : The participants encompassed ninety-nine participants aged above ۱۸ years. Theirmedian age was ۴۵.۹۲ ± ۱۳.۶۳ years. The most common complaints of the patients were epigastricpain and heartburn. Our described gold standard detected ۶۱.۶% and ۳۸.۴% as positive andnegative, respectively. The sensitivity and specificity were ۷۲.۱% and ۱۰۰.۰% for the routine PCR(glmM gene) and ۸۰.۳% and ۹۴.۷% for the proposed PCR (fliD gene).Conclusion : Different genes have been used to detect H. pylori in PCR. The glmM gene is easilyused to diagnose the H. pylori infection; however, according to the present findings, the fliD genehas higher sensitivity than the glmM gene. Accordingly, the former can be used as a screeninggene for the H. pylori infection in the PCR technique.
