Background and Aim : Mycoplasma pneumoniae is a common pathogen in community acquiredpneumonia (CAP) in childhood. This species affects the respiratory system. Macrolide resistancein Mycoplasma pneumoniae species is increasing in the world. Therefore, the aim of this study isto review the prevalence of macrolide resistance of Mycoplasma pneumoniae in children.Methods : We searched MEDLINE via PubMed, Scopus, Science Direct, Web of Science (ISI),Google Scholar (as English databases); Magiran, Iran Medex, Iran Doc, and SID (as Persiandatabases) during ۱۹۹۶ to September ۲۰۲۱using the terms: Mycoplasma pneumoniae pneumonia(MPP), community acquired pneumonia (CAP), children, Macrolide resistance.Results : Prevalence of MPP occurs worldwide in the age range of ۳ to ۷ years. Recent epidemicshave occurred in Korea. Although MPP is a mild and self-limiting disease, it can develop into asevere and fulminant disease. The prevalence of macrolide-resistant MPP is rapidly increasing,and has recently reached ۸۰–۹۰%, particularly in Asian countries. Macrolide-resistantMycoplasma pneumoniae (MRMP) has a point mutation in the V domain of ۲۳S rRNA, and mostof the mutations are detected at positions ۲۰۶۳ and ۲۰۶۴ of the sequence.Conclusion : Myoplasma pneumoniae strains show a good response to treatment with macrolides.Excessive use of macrolides may contribute to these mutations. MRMP can lead to clinicallyrefractory pneumonia, shows no clinical or radiological response to macrolides, and can developinto severe and complicated pneumonia in children. Tetracyclines or quinolones can be alternativesfor the treatment of MRMP.
