Factors Associated with Household secondary attack rate of COVID-۱۹ among health care workers

Background and Objective: Within-household transmission of SARS-CoV-۲ infection has been identified as one of the main sources of spread of COVID-۱۹ are implemented (۱, ۲). Given that the risk for transmission of COVID-۱۹ to persons with close contacts of infected persons, especially in health care workers has not been well estimated (۳). The aim of this study is to investigate the factors related to COVID-۱۹ household secondary attack rate among health care workers. Materials and Methods: The present prospective case-ascertained study was conducted on ۲۰۲ health care workers with confirmed COVID-۱۹ in Hamadan diagnosed during March to August ۲۰۲۰. For households with the close contact with the index case, RT-PCR was performed regardless of symptoms. We defined SAR as the proportion of secondary cases from the total of susceptible contacts that live in the household of index case. SAR was reported as percentage and ۹۵% confidence interval (CI). Multiple logistic regression was performed to explore the predictors of COVID-۱۹ disease transmission of index cases to their household.Findings: The study found ۳۶ secondary cases with laboratory confirmation among ۳۹۱ household contacts that represent a household SAR of ۹.۲% (CI: ۶.۳۳, ۱۲.۰۷). Among factors related to the family member, female gender (OR: ۲.۸۷, CI: ۱.۱۸, ۶.۹۴), being the patient's spouse (OR: ۲.۱۵, CI: ۱.۰۲, ۴.۵۵), and live in apartment (OR: ۲.۷۸, CI: ۱.۲۴, ۶.۲۳), and among factors related to index cases hospitalization (OR: ۵.۹۳, CI: ۱.۳۱, ۲۶.۹) and having caught symptom (OR: ۲.۳۵, CI: ۱.۰۷, ۵.۱۶) were the significant predictors of disease transmission to other family members (P<۰.۰۵).Conclusion: The findings of this study like other studies(۴, ۵), suggest the SAR in household contacts of infected health care worker is remarkable. SAR in household contacts is elevated in females, the patient's spouse, and live in apartment in relation to the family member and hospitalization and having caught symptom in index cases.
