The presence of obsessive-compulsive symptoms (OCS) and obsessive-compulsive disorders (OCD) in schizophrenia is frequent, and a new clinical entity has been proposed for those who show the dual diagnosis: the schizo-obsessive disorder. The prevalence of obsessive–compulsive symptoms (OCS) in schizophrenia patients is as around ۳۰%. Evidence suggested that mild OCS could reduce symptoms of schizophrenia, supporting the presence of compensatory functions. However, severe OCS could aggravate various impairments in schizophrenia patients, supporting the “double jeopardy hypothesis”. We present a case report of a ۳۴-year-old female patient with the schizo-obsessive disorder who first manifested psychotic symptoms in her early eighteen and in third time of her hospitalization, she manifested OCS. Hence, we provide a brief review on literature about this disorder. This case report suggests that a transition of psychotic symptoms in OCS, albeit rare, may seems to be modulated by environmental factors.
