Study of miR-۳۲۰a and its target genes IGFR and RAB۵β for prognosis of insulin resistance in women with polycystic ovary syndrome

Background and aim: Polycystic ovary syndrome (PCOS) is a common heterogeneous endocrine disorder. PCOS is associated with infertility, insulin resistance, and a low level of chronic inflammation. These patients are at risk for type ۲ diabetes. MicroRNAs are small noncoding RNA molecules that are involved in regulating gene expression. MiR-۳۲۰a had contradictory expression in PCOS; thus, we decided to evaluate miR-۳۲۰a and ۲ its target.Material and Method: In this case-control study, ۲۰ healthy controls, ۲۰ insulin resistance groups, ۳۰ patients with PCOS, and ۳۰ patients with PCOS and IR (PCOS + IR +) were included. None of the subjects had received any medication and none of them were pregnant. Expression changes of the genetic factors were measured by Real-Time PCR.Result: The PCOS + / IR + group showed a significant decrease in IGF-۱R and RAB۵β and a ۶-fold increase in miR-۳۲۰a. miR-۳۲۰a was decreased in patients with PCOS compared to the normal group, but a significant increase in miR-۳۲۰a was found in patients with insulin resistance (IR +). More than two-fold increase in IGF-۱R gene was observed in patients with PCOS, but its level was decreased in those with insulin resistance (in PCOS+ IR+ and IR+ groups). RAB۵β significantly decreased in all three groups of PCOS+, PCOS+ IR+ compared to healthy individuals, although the decrease was more pronounced in individuals with PCOS+ (PCOS+ and PCOS+ IR+ groups).Conclusion: All of these findings imply the pathological effect of miR-۳۲۰a on insulin resistance. miR-۳۲۰a may be suggested as a proposed biomarker or therapeutic target for patients with PCOS, and the reduction of the two genes studied has a direct relationship with the dramatic increase in miR-۳۲۰a in the development of diabetes. The ROC curve study showed the sensitivity and specificity of ۱۰۰% miR-۳۲۰a with IGF-۱R and RAB۵β for the detection of prognosis of insulin resistance. The results showed that these three indices could be used as candidates for diagnostic kits to predict the incidence of insulin resistance and ultimately type ۲ diabetes in women with PCOS+ and even healthy controls.
