Introduction: Presacral tumors are a rare group of hetrogenous lesions located in a Potential spacereferred to as retrorectal or presacral space. The estimated incidence is about 1 case in every 40,000hospital admissions. About two thirds of all the pre-sacral tumors are Congenital, and most of themare cystic and benign. Laparoscopy may be an alternative to the anterior abdominal approach.Case presentation: A 40 years old woman was referred to my hospital with complaints of infertilityand pelvic pain for 2 years, and large (12.5cm*11cm) solid –cystic presacral mass was reported byMRI and CT.The spinal involvement was not reported with MRI.Physical examination of the abdomen showed a surgical scar due to previous ovarian cystectomyabout 2 monthes ago, without palpable masses.Recto-vaginal examination showed a large massbetween the posterior vaginal wall and rectum.Labratory analysises were normal, including tumormarkers. The laparoscopic mass resection was performed, and final pathology reported, well –differentiated carcinoid tumor.Conclusion: Pre-sacral tumors are quite rare. Minimally invasive surgery gains more acceptancesrecently for resection of benign pre-sacral lesions.
