THE IMPORTANCE OF TIME MANAGEMENT IN TRAUMA PATIENTS AT PREHOSPITAL EMERGENCY CARE

Background and Aim : The majority of deaths caused by trauma in the developing world, occuroutside of the hospital. In patients with severe trauma, transportation to an emergency departmentwithin the ‘‘golden hour’’ is one of the most important markers of trauma system effectiveness.However, evidence regarding impacts on patient outcomes is limited.Methods : The data of this study, first were collected by searching keywords (on-scene time andoutcome parameters) in online research databases, including United States National Library ofMedicine (PubMed), Cochrane library and Google scholar. Then the results were groupedaccording to their date of publication and also strengths and weakness in their Material andMethods part. At the end, the content was integrated with more emphasis on stronger resources.Results : Results showing a decline in possibility of mortality for the trauma patients whenresponse-time or transfer-time getting shorter. On the opposite, increased on-scene time and totalprehospital time are associated with increased possibility of survival for this population. However,rapid transport does seem beneficial for patients suffering penetrating trauma, in particula rhypotensive penetratingly injured patients and patients with a traumatic brain injury.Conclusion : Rapid transport is beneficial for patients suffering head injuries and thehaemodynamically unstable penetratingly injured patient. Moreover, in haemodynamically stableundifferentiated trauma patients, increased on-scene-time and total prehospital time do notincrease the possibility of mortality. Also for undifferentiated trauma patients, it is necessary tofocus on the type of prehospital care delivered, not on rapid transport.
