Tuberculosis or sarcoidosis: a diagnostic dilemma

چکیده مقاله:

Introduction Tuberculosis (TB) is a common infection in developing country and its manifestations are likely similar to sarcoidosis. It is a challenging situation for diagnosis.in this report we presented a patient with tuberculous sarcoidosis Case presentation A 27 years old female referred to hospital with fever also she had headache, weight loss and night sweating for 1 month. In imaging evolution there was a mass and lesion in brain. Biopsy of lesion showed non-caseating granulomas, also Acid fast and PCR of it was negative. So, sarcoidosis treatment was started, but her symptoms were relapsed for two time in a year. More evaluation was done and BAL’s results was significantly positive for acid fast test in favor of TB infection. Complete recovery was seen after anti TB drugs.Discussion TB and sarcoidosis are same disease and differentiate is challenging. Several studies demonstrated detection of TB DNA in sarcoidosis tissue. This case and some other reports similar to ours support this idea that, there is the lack of a diagnostic accurate test of these two diseases.
