Introduction Guillan Barre syndrome is a rare neurological involvement in patients with SLE. we report a patient with type IV lupus nephritis whose clinical coarse complicated with Guillain Barre syndrome.Abstract Forty two years old man who was a known case of type IV lupus nephritis was admitted with chief complain of edema and some dyspnea. He was on prednisolone 50 mg/d and cyclophosphamid pulse monthly. He developed more dyspnea and his creatinine raised up to 5mg/dl, EF about 20%. He was admitted on ICU and underwent hemodialysis. He received 5 methyl prednisolon pulses (1000 mg/d) follows by 1000 mg cyclophosphamid at end. His general condition improved, but he said that he cannot move his legs. After neurological consult and NCV evaluation with diagnosis of Guillain Barre syndrome he received plasmaphresis 5 times every other day and partially improved. His clinical coarse was complicated with episodes of catheter infection and pneumonia and he recieved broad specterum antibiotics. he was discharged on oral prednisolone and hemodialysis three times weekly after about 70 days admission.Discussion lupus nephritis is a uncommon pathology in male patients, our patient developed a rare neurologic complication of lupus when he was on full dose immunosuppression.
