Introduction:: Constrictive pericarditis is a rare disease . it is caused by fibrosis of the pericardium. Clinical signs and Symptoms are often ambiguous and the diagnosis is challenging because it is necessary to differentiate this pathology from other reasons of right heart failure. The majority of cases are of unknown etiology but in some patients, Pericarditis may occur with rare causes .Case Presentation: We report a 22 years old male patient .he referred to our hospital with dyspnea ,lower extremely edema ,coughing and sputum .he had no family history of any illnesses but he reported two periods of
TB and taking RIPE during 7 months. After this period, the patient has arbitrarily stopped taking the medication but after two weeks ,generalized edema was occured and the patient developed abdominal ascites. We immediately requested the necessary tests . The results of blood tests were normal and sputum culture showed no pathology. So ECHO obtained at the request of physician . The results had shown increased PAP (45-50mmHg) and pericardial thickening .(11mm). All the above confirmed a diagnosis of
constrictive pericarditis .finally total pericardiectomy was confirmed for him and the tissue PCR was significant for positive Mycobacterium tuberculosis, Because of the
TB tests had already been negative.Discussion: The case we reported was a
constrictive pericarditis due to Mycobacterium tuberculosis.
tubercolous pericarditis accounts to 1-2% of tuberculosis patients. An entity that has become frequent .
tubercolous pericarditis is a rare diagnosis but remains common cause of pericarditis. also Tuberculosis treatment takes at least 6 months but it may take a longer time. A patient should be well educated about medication use and does nt stop taking the medication arbitrarily ,because it will cause dangerous side effects like the pericarditis that we mentioned.
