The first experience of radioligand therapy with 177Lu-PSMA of mCRPC patients in IRAN

Background: The purpose of this study is to evaluate the effect of radioligand therapy with 177Lu-PSMA in mCRPC (metastatic castration resistant prostate cancer) patients.Methods: 17 mCRPC patients with average age of 73.47±8.97 who had no other treatment options were selected for radioligand therapy with 177Lu-PSMA between November 2016 and August 2018.Results: The 17 patients were treated with a total number of 34 cycles (range 1-5) of 177Lu-PSMA (total dose 219 GBq, median dose 5.91 GBq, range 4.77-7.4 GBq). 70% of the patients showed any amount of PSA decline after the first cycle. 5 patients showed PSA decline ≥50% and 7 patients had PSA decline <50%. Disease progression occurred in 5 patients. 13 patients underwent the second cycle of therapy unfortunately data for 7 patients were available. PSA decline was observed in 5 patients (71.5%), 2 of them showed a decline ≥50%. Two patients showed progression. No kidney and liver toxicity were noted. One patient had grade III anemia and two patients had grade III thrombocytopenia. Three patients died in the follow up after the first cycle and five patients died after the second cycle.Conclusion: Radioligand therapy with 177Lu-PSMA in mCRPC is effective and safe
