Recurrences of cutaneous Leishmania in a middle-aged woman caused by reinfection, not relapse: a case report

Background: Leishmaniasis is an intracellular parasitic infection transmitted to humans by the vector sand fly, infects about 12 million people globally and threatens 350 million more, human populations in Leishmanasis endemic areas are exposed to suffering repeated infections. Although both Leishmaniasis reinfection and relapse of Leishmaniasis are associated with immune deficiency but they are different. Relapse Leishmaniasis is reactivated infection, whereas reinfection means secondary infect, by secondary bite and often in secondary site.Objectives: This study is report of a cutaneous Leishmaniasis recurrence in a middle-aged woman.Case report: A 50-year old female was presented with the history of reinfection at the outpatient clinic of skin diseases and Leishmaniasis research center of Isfahan. She suffered from Leishmaniasis in childhood. After a few years she has come back with a new lesion in face that physical examination showed Leishmaniasis lesion (12mm in diameter) in the form of plaque on the tip of the nose with a positive smear. After sampling and microscopic examination Leishman bodies were observed in all the samples of wounds. Lishmaniasis was diagnosed and then local treatment with glucantime was initiated immediately.Discussion: Based on the studies in Leishmaniasis-endemic area, however the exact cause of the reinfection is no clear, but strain variations, reduce immunity over aging, incomplete treatment of Cutaneous Leishmaniasis, underlying diseases predisposing for reinfection are the main reason. In this report, person reinfected with Leishmania was probably due to the history of migration to an endemic area and incomplete treatment that has caused the immune system was not well stimulated previously to provide a correct response to recurrent cutaneous Leishmaniasis.Conclusion: In fact, the presence of Leishmaniasis reinfection indicates that Leishmaniasis is not completely immune, so a vaccine for Leishmaniasis has been a goal for years, but there is still no effective vaccine, Studies on the immunogenicity of Leishmaniasis are ongoing, and researches on reinfected individuals may be helpful in examining the process of immunization
