Isfahan Diabetes Cohort Study: a Prospective Study on Prevalence and Incidence of Diabetes and Prediabetes in First-Degree Relatives of Type 2 Diabetic Patients

Introduction: There is a lack of information on the probabilities to transit from normal glucose tolerance (NGT) to different pre-diabetic states and from these states to T2DM. The objective of our study is to estimate the risk equations and to quantify the influence of single or combined risk factors on these transition probabilities.Materials & Methods: This is a prospective cohort study that has been started since 2003 and is being continued. 3206 first degree relatives of diabetic patients aged 35-55 years were registered into the study from 2003 to 2012. Participants have been categorized into 3 groups; norm-glycemic, pre-diabetes and diabetes. Norm-glycemic and pre-diabetic groups have been followed during the study. Dietary assessment, physical activity, anthropometric measures, glycemic and lipid profiles, liver function tests, CBC, uric acid, and blood pressure were collected at the beginning of the study and have been repeated every three years in NGT and every one year in prediabetes groups. The data including outcome and base characteristics are double-entered in SPSS software to secure its validity. Preliminary analyses and transformations of variables will be analyzed using a linear mixed model analysis of variance (ANOVA) with a between-subject factors approach which will allow us to make inference about time effects in each studied group.Conclusion: The results of this study increase our knowledge impact on the trend of healthy /pre-diabetes to T2DM and planning for the early prevention of T2DM. Moreover, we can obtain precious information about clinical features associated with the progression of diabetes. It would be interesting to organize follow-up to improve the prediction model, not only diabetes but also cardiovascular outcome and will serve to clarify the question further.
