Introduction:Motor vehicle accidents account for 90% of urethral injuries. Urethral trauma in females is rare. The usual mechanism of injury involves pelvic fracture incurred during a motor vehicle accident. The management of urethral injuries depends on if the rupture is complete or partial as well as the timing of surgical intervention.Urethral crush injuries that do not involve the bladder neck are managed by extended transurethral Foley catheterization (6 to 8 weeks) or, if necessary, suprapubic catheter drainage. Case presentation:3.5 year old female, multiple injuries followed car accident that in another center without attention to external genitalia trauma underwent laparatomy and splenectomy. 48 hour after operation referred to us for urine retention. Examination revealed severe laceration of the anterior vaginal wall with complete urethral avulsion Urethral plate and vagina were completely detached; that repaired with indwellingurinary catheter.Conclusion:Maltreatment of children leads to significant morbidity in them. Early recognition and proper management with primary urethral and vaginal repair can prevent the subsequent risk of complications
