In Iran, similar to other Islamic and Jewish societies, all boys are circumcised in observance of religious and/or cultural traditions. These circumcision procedures are commonly performed by pediatricians, general practitioners, obstetricians, and general surgeons, and to a lesser extent, by other medical specialists. In rural and remote regions, often non-specialized, poorly trained individuals perform circumcisions.For practical or economic reasons, most circumcisions are performed under local anesthesia in outpatient clinics, or medical offices. These procedures are either performed by plastibell or various surgical techniques. The former is much more common. As part of our own surgical practice we provide circumcision services for children. From the perspective of penile adequacy, we feel that the cosmetic result of our circumcision technique is superior compared to those commonly performed by others. We have developed a criterion for circumcision,which removes only the foreskin preserving the penile shaft native skin, such that the appearance of the penis is not distorted and hence the primary goal of circumcision is achieved. Our criterion for proper circumcision is to remove a defined amount of foreskin in order to expose the glans whilst preserving maximum length of penile shaft native skin. This ensures the health of the penis and allows it to developnaturally without risk of unsightly appearance or adverse functional consequences.
