Unusual presentation of foreign body ingestion in a 6-year old boy

Introduction:Of more than 100000 case of foreign body ingestion usually occur in children aged less than 3 years. Most foreign body that reaches the gastrointestinal tract pass spontaneously. 10-20% requires endoscopic removal and 1% require surgical intervention. Mortality from foreign body is low and death is reported. Most foreign body include coins-button-batteries-toys-magnets-screws-marbles and bones.The most common sharp objects by children are straight pins, needles, papers, clips and fish bones.Sharp foreign objects have a high risk of perforation. Long objects that are long and blunt are ingested by older children or adult. The approach to management depends on the length and other characteristic of the object.Case Presentation:A 6-year-old boy with a complaint of upper abdominal pain after every bending position from a week ago that has intensified one day ago referred to the department of pediatric surgery. Abdominal pain was presented by bending forward every time. Abdominal ultrasound was normal. In abdominal CT scan and abdominal X- Ray one foreign body was observed corresponding to the duodenum.The patient was undergone surgery. Foreign body was in the C-loop of duodenum. The foreign body was pushed into the stomach and removed by gastrostomy. Foreign body was 11 CM long and it was metal (toy car antenna). And the child never had a history of foreign body ingestion.Discussion:The nature of the children is to take objects into their mouths and frequent cause for emergency department visit. But approximately 95% passes through the gastrointestinal tract. Ingestion of foreign body can lead to serious damage to the gastrointestinal tract.The most of this foreign body may mislead the clinicians. We report the case of a 6 year- old boy who ingested a toy car antenna length of 11 centimeters which was being located in duodenum. Conclusion:In general, if foreign body stops progressing parts of gastrointestinal tract, it should be removed as soon as possible by surgery specifically in case of sharp and pointed object.
