Esophageal Causative Injury, treated with colon interposition; A case report

Ingestion of corrosive substances remains a major health hazard in children. Most corrosive substances ingestions occur in children younger than 3 years old. Boys are more frequently involved. Although mortality is rare, morbidity is often devastating and is associated with lifelong consequences. Full thickness injury to more than a short segment of the esophagus invariably results in an intractable stricture which fails to respond to dilatation for longer than 6-12 months. The management of esophageal strictures due to caustic injury has evolved from the endoscopic dilation to surgical treatment. Types of esophageal replacement include: - Colon interposition - Gastric tube esophagoplasty- Jejunal interposition - Gastric transposition Colon interposition continues to be a widely used procedure for esophageal replacement in children. We report a 2 years old boy with long segment stenosis of the esophagus resulting from accidental ingestion of strong alkaline liquid.Result of first esophagoscopy was severe esophagitis in all esophagus length and severe gastritis. After unsuccessful esophagoscopic dilation for 6 month, Due to progressive dysphasia, it was decided to esophagectomy and colon interposition.After left colon interposition and 2 years follow up, the patient had a good growth without significant complications.
