Assessing the Preparedness of Health Care Centers against Disasters and Emergencies: A Case Study of Damghan County, Iran, 2018

Background and Aim: Iran is exposed to a wide variety of natural and man-made hazards. In emergencies and disasters, a community must be able to protect the lives and well-being of the affected population, particularly in the minutes and hours immediately following impact or exposure. Health care centers can play an essential role in providing lifesaving measures during these situations. The aim of this study is to evaluation the preparedness of health centers in Damghan county, Semnan province. Methods: This cross-sectional study was conducted in 2018. The samples were all the eleven health care centers which were located in Damghan. We developed a checklist as the collection data tool based on Hospital Safety Index (HSI) and disaster risk assessment checklist in Iranian primary health care facilities. Also, the observations in the field were used for completing the checklist by researchers. The checklist included 225 questions and divided into four sections: understanding threatening hazards of health care center, functional safety, structural safety, non-structural safety that were evaluated 42, 28, 5 and 7 components, respectively. Each question was evaluated with three safety levels: low, moderate, and high. Finally, the quantified risks were prioritized and control measures proposed accordingly. The descriptive and analytical statistical tests were used for analyzing data by STATA software, version 11.Results: The results showed that the health care centers understudy was exposed to 22 different natural and man-made hazards in northern and southern regions of county. The overall preparedness of the eleven health care centers in this study was 45%. The average of functional, structural, and nonstructural safety was assessed 40.58%, 45.12%, and 50%, respectively. The components of disaster training courses and doing exercises had the minimum scores of functional safety.Conclusion: The finding of this study showed relatively average preparedness of the health care centers understudy in Damghan County. Expanding the disaster educational programs and tabletop exercises are recommended among health staff in order to increase the functional safety and preparedness in the healthcare centers at time of emergencies and disasters.
