Beneficial effects of L-arginine supplementation in pre-diabetic subjects: A randomized, double blind, placebo controlled clinical trial

Background and Aims: limited studies investigated the effect of L-arginine supplementation on glycemic indices of pre-diabetics subjects who were obese or had metabolic syndrome. This study was undertaken to investigate the effect of L-arginine supplementation on body composition and glycemic indices of pre-diabetic subjects.Methods: This randomized clinical trial was conducted on 42 pre-diabetic subjects. Participants randomly received L-arginine supplement (n=21, 3 g/d) or identical inert placebo (n=21) for 8 weeks. Daily dietary intake, anthropometric measures and glycemic indices were determined at the beginning and after an 8-week intervention period. Results: Thirty-eight patients fulfilled the study protocol. After adjusting for changes in percent of daily fat intake, body mass index, serum L-arginine and baseline values, L-arginine supplement significantly decreased serum fasting glucose (FG), increased pro-insulin and the quantitative insulin-sensitivity check index (QUICKI) (p<0.05). There was no significant difference between two groups in term of 2-hours oral glucose tolerance test (OGTT), serum insulin concentration, the percentage of HbA1C, the homeostasis model assessment-estimated insulin resistance (HOMA-IR) index and beta-cell function values. Conclusion: An eight-week intake of L-arginine supplement could significantly improve some parameters of glucose metabolism and insulin sensitivity in pre-diabetic subjects.
