Introduction & Aims: Sarcoma of the pancreas is comparatively rare. Pancreatic sarcomas tend to be more aggressive and have a poor prognosis. Methods: The case of a 51 years old patient presenting with jaundice, is presented, a CT-Scan of abdominal and abdominal ultrasound revealed a large process of the pancreatic head. The diagnostic workup revealed elevated of liver function tests and serum bilirubin (cholestatic pattern). An ERCP was performed but it was unsuccessful then abdominal laparoscopic was indicated. Histologic sections revealed undifferentiated pleomorphic sarcoma. A stain for CD68 was positive but CD30, CD34 were negative. .The patient was referred to oncologist and now is under chemotherapy treatments. Results: In this case, CT-Scan of abdominal and abdominal ultrasound revealed just a process of pancreatic head and, ERCP was unsuccessful, then by abdominal laparoscopic and pathology of biopsy , Primary Sarcoma Of The Pancreas was diagnosed. Sarcomas are rare human malignancies and primary pancreatic sarcomas represent an extremely rare histopathological entity accounting for less than 0.1% of all pancreatic malignancies. Conclusion: Pancreatic sarcomas are more aggressive than other pancreatic neoplasms; they’re associated with a worse prognosis.
