Exaggerated pseudoepitheliomatous hyperplasia after plantar wart surgery,a case report

Pseudoepitheliomatous hyperplasia(PEH)is a benign ,reactive and uncommon condition characterized by hyperplasia of the epidermis and adnexal epithelium and it closely mimics squamous cell carcinoma(SCC).PEH may be present in a number of conditions ,such as:prolonged inflammation and irritation,chronic infection,adjacent to an ulcer,trauma and in association with many cutaneous neoplasms.We report an exaggerated and giant cutaneous lesion in a 45-year-old woman who had history of plantar wart.She has been suffering from severe infection and osteomyelitis after she had been undergoing plantar surgery.Several times she had been admitted to the hospital and had undergone surgical debridement.Then skin graft for her was done.But the lesion was reformed after grafting and gradually forming raised and cauliflower appearance.The lesion was excised.Microscopically the raised lesion showing florid pseudoepitheliomatous hyperplasia with marked acanthosis,hyperkeratosis and parakeratosis of epidermis.No human papilloma virus(HPV)cytopathic effect was seen.There were no dyskeratosis or nuclear atypia .Special stain for fungi was negative.SCC is the most imprtant differential diagnosis of PEH,and differentiation of them is very imprtant to avoid possible unnecessary surgery.Due to exaggeration of pseudoepitheliomatous hyperplasia and resembling to SCC ,we decided to report this rare case.
