Abstract: Today, we are frequently hearing some complaining reports on unreliability laboratory results. This subject can be due to no harmonization of analytical issues that cause misinterpretation of the results or might be cause of some other problems. Despite nationwide efforts to develop and implement the quality assurance, at present there is no independent study available on the extent of success and weakness of current Quality Assurance program for laboratories. The health reference laboratory (HRL) and general directorate of laboratory affairs in each medical sciences university control the quality of diagnostic tests, check compliance with safety guidelines and standards for all clinical laboratories.Clinical laboratory results conform to expectations for the quality required for patient care. They tend to be focused on very narrow concepts of EQA, even though the significance of pre and post laboratory errors is now widely recognized. External quality assessment and Proficiency testing have been and should remain an integral part of laboratory quality assurance. These two programs should indeed identify systematic errors in methodology that may not be revealed by internal QA processes. Auditing program is one of the main strategies to monitor and improve the quality. Despite having a potential critical role in improving quality of clinical care, scientific evaluation of audits has not be done or is not being reported sufficiently. It seems there are some potential barriers in the correctly execution of audits across the laboratories. Besides, regular performance of internal audits is frequently ignoring. On the other hand, the incapability of the current running EQA schemes is frequently discussed, especially its ability to signifying the weaknesses of pre-analytical, analytical, and post analytical laboratory processes. Therefore, revision on Strategy of Quality Assurance Program may be a way, but before taking any efforts current program must be evaluated by the independent body.
