Diagnosis of Helicobacter pylori infection and association with histological findings in patients with peptic ulcer and gastritis

Background and Objective: Helicobacter pylori (H. pylori) is accepted as the most important agent of gastritis and as a risk factor for peptic ulcer disease and gastric adenocarcinoma. The accurate detection of H. pylori is essential for managing infected patients. The aim of this study was to diagnosis of H. pylori infection and its association with histological findings in patients with peptic ulcer and gastritis.Material and Methods: This study was performed on 228 dyspeptic patients who referred to Amir-al-Momenin Ali Hospital in Gerash (Fars Province, Iran) from March 2017 to February 2018. The study excluded those patients who were treated for H. pylori infection, were used Proton pump inhibitors (PPIs) and H2 blockers. Three biopsy specimens were obtained from the gastric antrum, gastric body and angular incisure for each patient. Biopsy specimens was used for Rapid Urease Test (RUT) and fixed in formalin 10% and stained with Hematoxylin-Eosin (H&E) and Giemsa for histological examinations. Chi-square and fisher’s exact tests was used for data analysis. Results: Among 228 patients, 85 cases (37.28%) were infected with H. pylori. The mean age of patients was 45.1 ± 21.3 years. 40 patients (47.05%) were female and 45 patients (52.94%) were male. There is not significant relation between age and sex with H. pylori infection (p> 0.05). 52 cases (61.17%) of chronic gastritis, 13 cases (15.29%) of intestinal metaplasia, 9 cases (10.58%) of atrophic gastritis, 8 cases (9.41%) of chronic gastritis with intestinal metaplasia, 3 cases (3.52%) of lymphatic infiltration were seen. Dysplasia and gastric cancer were not seen. Conclusion: Cannot assume cases as H. pylori positive only by relying on diagnosis of peptic ulcer and gastritis. The rapid diagnosis of peptic ulcer and gastritis due to H. pylori is very important because the chance of stomach cancer will decrease after treatment.
