Burn is a serious occurrence that affects the affected person, family, community and nations of the world. Extreme burns create major constraints and prevent a productive life for its victims. The process of transferring a patient from a hospital to a patient and his family and his carers is complicated and difficult due to initial care after discharge. Follow-up care is an important element of post-clearance care that can be done through home visits and tele-nursing. The benefits of such care include frequent relief from burn centers, cost reductions, increased satisfaction, and lack of additional care. Encouraging patients to receive complementary therapies. In Iran, despite the benefits of home care, community health services are still not as good as advanced hospital services. When patients are discharged from a hospital, they have a high level of support for the medical, nursing and specialist system. They would get into an environment that is with official caretakers Or unofficial visits to home nurses. They are largely abandoned by confusion and controversy among doctors and pre-discharge advice, incomplete understanding of service providers from the home environment and their abilities. Between the nurses at home care, the hospital, post-clearance care is not sufficiently formed. Generally, doctors who have been serving the patient in a hospital are often not associated with nursing home care, and to the nurse s needs and patient follow-up process. They are not very aware. In general, there is no basic and standard information in this area.
