Education is fundamental to Addison’s disease pregnant woman and their caregiver: a case report

چکیده مقاله:

Addison s disease (AD) is primary adrenal insufficiency that associated with cortisol and mineralocorticoid deficiency. Adrenal crisis (AC) occurs in patient with pregnancy, acute major mental/physical stress, serious infection, surgery, adrenal infarction, gastrointestinal disease and under replacement of medicine.34-year-old women, known case of Addison’s disease that diagnosis was establish 2 years ago. When she had 30 years became hypothyroid and 32 years AD diagnosis was confirmed. After seven years of infertility, became pregnant without induction-ovulation (2years after treatment of AD). Cesarean section was done because she had labor pain and breech presentation in 38/70 weeks. She gave birth a healthy girl with APGAR score at 1-5 minutes was 9-10. Neonate’s weight was 2620 gr. Vasodilatatory shock is the main presentation of AC that is a life threatening condition and needed immediate treatment. The goal of this presentation to clearance the education to patient and partner/caregiver is very important and essential in treatment of AC emergency situation.Conclusion: All adrenal insufficiency patients should have their own medical information card, glucocorticoid (100 mg hydrocortisone or 4 mg dexamethasone) and normal saline 0.9% for injection in emergency situations.Finally when we visited the pregnant patient with AD, except give reassurance and meditation to reduce her stress, should be trained, she and her partner/caregiver about self-adjustment and self-injection of medicine.
