The effect of Lasix on controlling hypertension after the C-section of patients afflicted with mild preeclampsia: a double-blinded clinical trial

Background and Purpose of the study: There is limitation for the consumption of diuretics during pregnancy to control hypertension. However, taking furosemide as a diuretic in controlling blood pressure has been successful. Thus, the present research aimed to explore the effect of furosemide on controlling postpartum hypertension in patients afflicted with mild preeclampsia.Materials and Methods: The present research was a double-blinded clinical trial in the Khalij Fars Hospital of Bandar Abbas in 2017. After the C-section, cessation of magnesium sulfate and occurrence of diuresis, patients’ systolic and diastolic blood pressure was checked and the patients consented to enter the study according to the inclusion and exclusion criteria. They were randomly divided into two research groups one treated with oral furosemide (20 mg) on a daily basis for 5 days and the other receiving the same amount of placebo.Results: The mean age of the furosemide group was 32.55±7.93 years and the control group 30.96±8.20 years (p=0.239). The mean weight of the former was 62.75±11.02 kg and the latter 63.41±11.51 Kg (p=0.756). The systolic and diastolic blood pressure as well as MAP were reduced in both groups 1 day after the treatment. However this reduction was more in the group treated with furosemide (p˂0.05). By the end of the seventh day, the two groups showed to diverge significantly in terms of the systolic blood pressure (p˂0.05). On the third, fifth and seventh days, the two groups showed no statistically significant difference in terms of diastolic blood pressure (p˃0.05). The mean MAP continued to reduce three days after the treatment. Moreover, the rate of reduction in the treatment group showed to be higher than the control and this difference was statistically significant (p=0.013). However, on the fifth and seventh days, the mean difference of MAP was not statistically significant in the two groups (p˃0.05). Conclusion: In the light of the present findings, patients afflicted with mild preeclampsia who continue to have a hypertension after childbirth are recommended to take oral furosemide (20 mg) on a daily basis after the cessation of magnesium sulfate and occurrence of diuresis.
