Background: During the last decades, apo-lipoproteins has attracted many attentions because of their roles in transporting the lipids. Apo-lipoprotein B (Apo B) transports very low‐density lipoprotein (VLDL), LDL particles and intermediate‐density lipoprotein (IDL), and apoA‐I has a role in transporting HDL particles. In this study we discuss the significant role of the apoB/apoA‐I ratio in cardiovascular disease.Methods: We did a search based on the keywords Apo-lipoprotein B AND Apo-lipoprotein A-1 AND Cardiovascular disease in PubMed, EMBASE and Google Scholar databases. We found 54 articles that 31 of them are related to our subjects.Results: We investigated the results of the prospective studies in healthy individuals and in patients with various clinical signs of atherosclerosis. According to the reports, risk of incidence with stroke, and signs of atherosclerosis recognized by diagnostic methods such as ultrasound, angiographic, has been associated with Apo-lipoproteins. The cholesterol balance proved that the apoB/apoA‐I ratio is a better marker than lipoproteins and lipid ratios.Conclusions: The results show that the apoB/apoA‐I ratio is an easy, precise and novel risk factor for cardiovascular diseases (The low apoB / apoA-I ratio causes the reduction in cardiovascular risk). The guidelines should be developed to identify the important information of clinical risks that embedded in the apoB / apoA-I ratio.
