rare case of solitary plasmacytoma of maxilla and review of the literature

مقدمه: Plasma cell neoplasms are discrete, solitary masses of lymphoid neoplastic proliferations of cells. Solitary bone plasmacytoma(BP) and extramedullary plasmocytoma are unifocal and monoclonal proliferations plasma cells, whereas, multiple myelomas are the multicentric and disseminated type of this disor der. Due to variable biological behavior, these tumors can have periods of clinical latency to rapid growth and progression from the localized forms to multiple myeloma. معرفی بیمار: 59 year old male patient was referred to school of dentistry with chief complaint of painful mass of upper jaw for months. Intraoral clinical examination revealed cm mass with well-defined borders and intact surface located in the posterior part of left maxillary bone. After the microscopic and immunohistochemical evaluations of the incisional biopsy, the diagnosis of plasmacytoma of maxilla was made. Clinical and laboratory investigations confirmed that it is SP of jaw. The patient underwent four sessions of chemotherapy and 10 sessions of radiation with planned dosing of 40-50 Gy. He has been followed up for months after the treatment. The tumor had regressed completely and the laboratory results remained within the normal limits. بحث نتیجه گیری:The diagnosis of SP is difficult and might be misdiagnosed as odontogenic and less frequently other malignant tumors in jaw. Criteria for diagnosis of SP includes single lesion histologically investigated, negative skeletal survey and tumor-free bone marrow. Radiation therapy, radi cal extensive surgery or both are the primary treatments and surgery is the first choice. Surgical treatment is not the preferred treatment in most instances and is mostly reserved for patients with well-defined SPs and minimal cosmetic or functional deficit. Local radiation and chemotherapy may delay the process of SP converting to multiple myeloma. According to this case report, we intend to show that how early diagnosis is crucial to cure the disease with the right treatment plan.
