Background: Postpartum hemorrhage is one of the main causes of maternal morbidity and mortality in developing countries. Oxytocin is conventionally used for uterine atony during cesarean section. This study was performed with aim to evaluate the effect of combined oxytocin and sublingual misoprostol in reducing blood loss during and after cesarean delivery.Methods: This randomized clinical trial was performed on 90 pregnant women with term pregnancy who were candidate for emergent cesarean delivery with spinal anesthesia and were high risk for post partum hemorrhage. All participants received 40 IU oxytocin in 1 liter of normal saline after delivery. Participants were randomly assigned to receive 400 μg misoprostol and oxytocin infusion or only oxytocin infusion. Data was analyzed by SPSS software (version 23). P<0.05 was considered statistically significant.Results: Mean postoperative blood loss was significantly lower in the misoprostol group (P<0.001). pyrexia and other side effects were similar in both groups.Conclusion: Combination of sublingual misoprostol and oxytocin infusion seems more effective to reduce blood loss 6 hours after cesarean section among women with risk factors for PPH.
