Abscess Formation After Laparoscopic Management of Multiple Ovarian Mature Cystic Teratomas with Adhesion to Bowel:A Case Report

Mature cystic teratoma (dermoid) is the most common type of benign neoplasm of the ovary. Abscess formation after cystectomy constitute a very unusual report. We report a woman with a huge abscess after ovarian cystectomy detected by ultrasound and CT Scan. It was drained under ultrasound guide. A 36-year-old fatty woman with the history of two cesarean section and concomitant left oophorectomy due to multiple ovarian tumor admitted to a teaching hospital with abdominal pain. Ultrasonography showed multiple ovarian cystic mass suspicious for dermoid in the right ovary. On laparoscopy, left ovary was absent and uterus was normal, while right ovary presented ten dermoid cysts with adhesion to sigmoid and small bowel. All cysts were enucleated and removed laparoscopically. The patient had an uncomplicated recovery and was discharged two days after surgery. She attended to hospital with abdominal pain and fluctuating fever after one week. Spiral CT Scan showed a huge cystic formation suspicious for Abscess. She was treated with drainage under ultrasound guide and antibiotic through ten days and discharged with no complaint. Bowel injury due to adhisionolysis and absence of aseptic surgery were probable cause. Pathology confirmed the diagnosis of multiple mature cystic teratoma.
