Whether Serum Ferritin Level Impact on GvHD and Survival in the Patients Undergoing Hematopoietic Stem Cell Transplantation

Background and Aim: Hematopoietic stem cell transplantation (HSCT)and graft-versus-host disease (GvHD) have a complicated biologicalnetwork which can be related to many biofactors. Serum ferritin is anacute phase protein and an indicator for iron storage. Iron overload canlead to liver damage, hepatic sinusoidal obstruction syndrome, infectionand other complications which substantially influence HSCT outcomes.Thus, we performed this study to investigate the prognostic significanceof pre-transplantation serum levels of ferritin on the incidence of GvHDand overall survival in patients undergoing allogeneic HSCT.Methods: In this study, we retrospectively evaluated the data (serumlevels of ferritin, GvHD status, and survival) from 196 patients withhematological malignancies who underwent allogeneic HSCT inTaleghani bone marrow hematopoietic stem cell transplantation center,Tehran, Iran. We examined the association and correlation of pretransplantationserum levels of ferritin with GvHD and overall survival of patients. The data were analyzed using Kaplan-Meier and Cox regressiontests.Results: Our results showed that 5-year overall survival (OS) is significantlyhigher in the patients with pre-transplantation serum ferritin levels under800 μg/L (68% versus 62%, P value= 0.001) than the patients with serumferritin more than 800 μg/L and there was a negative correlation betweenserum ferritin level and OS. In addition, it was shown that the pretransplantationserum ferritin level is higher in the patients with GvHDbut there was no significant correlation between serum ferritin level andGvHD (P> 0.05).Conclusion: This study confirmed that high serum level of ferritin beforeHSCT might be associated with survival rate after allogeneic HSCT andincreased pre-transplantation serum levels of ferritin can be a predictorof HSCT outcomes.
