The Serum Levels of Uric Acid Might Be a Predictive Biomarker for Acute GvHD

Background and Aim: Acute graft-versus-host disease (aGvHD) continuesto be the major complication of allogeneic hematopoietic stem-celltransplantation (AHSCT). The diagnosis of aGvHD is dependent onclinical manifestations which mostly occur after organs damages. So,identification of predictive biomarkers can be useful to prevent andmanage aGvHD. Following conditioning for AHSCT, danger signals suchas uric acid (UA) are released by the affected tissues but the associationof UA levels with aGvHD is still controversial. This study investigated theassociation of serum levels of UA with the incidence of aGvHD.Methods: Altogether, 196 patients with hematological malignancies,who were candidates for AHSCT, were randomly chosen and 2 mLperipheral blood samples were taken with signed consent form at the dayof the transplantation (day 0). The serum samples were separated fromwhole blood and the UA serum levels were quantified using enzymelinkedimmunosorbent assay (ELISA). The occurrence of aGvHD in 100days post-transplantation was evaluated using clinical manifestation inaccordance with NIH criteria. The results were analyzed using SPSS.Patient survival was calculated by using the Kaplan–Meier method.Results: Overall, 47 patients out of 196 recipients (24%) developedaGvHD. The UA serum levels of aGvHD patients was significantlylower than those of recipients without aGvHD. (P value = 0.0051, riskratio=0.713 ). There was no significant association between UA levelsand survival.Conclusion: Our results suggest that the UA serum levels of patients atthe day of transplantation might be a predictive biomarker for occurringaGvHD so that patients with lower UA serum levels are probably moresusceptible for aGvHD. Determination a cut-off for this parameter canhelp for better prediction.
