Background and Aim: Vascularized and non-vascularized fibular grafttransfer for skeletal reconstruction purposes has been shown to be aneffective treatment strategy for various congenital and acquired skeletaldeformities, including for mandibular reconstructions due to tumor,infection and trauma.Methods: We present a novel method where free fibula transfer utilizingpreoperative virtual surgical 3D planning is used to produce a totalautogenous reconstruction.Results: The virtual surgical planning allows to effectively quantifythe bone stock required preoperatively. Moreover, the virtual surgicalplanning is used to carefully design nylon saw guides that contain alsopre-planned drill holes for screw placement, which can be used duringsurgery to optimally cut the fibula such that an anatomically correctreconstruction can be realized.Conclusion: This will result in reduced surgery times and improvedfunctional and aesthetic outcomes. In the near future, this type ofreconstruction may be replaced by using personalized 3D-printedscaffolds seeded with adipose stem cells to avoid donor site morbidities.
