Background and objective: TMS (transcranial magnetic stimulation) is a non-invasive neuromodulation technique for treatment of depression , and it has also been evaluated in humans for the treatment of tinnitus and has a very direct influence on brain physiology. The magnetic pulse stimulation may break up synchronous activity in the brain associated with the perception of tinnitus.Search method: in this review article management of chronic tinnitus with rTMS was considered. pubMed, science direct, text book of tinnitus, google scholar, treatment text book search was performed.Findings : researchers for recovering the affect of rTMS prepare two groups real (rTMS group) and sham. Daily low-frequency rTMS to the left temporoparietal junction area for 2 or 4 weeks was no more effective than sham for patients with chronic irritable tinnitus . Possible explanations for the negative findings includes short duration of treatment or maybe because of the failure of rTMS to stimulate deeper parts of auditory cortex within the sylvian fissure. The studies did not recover any changes in neural connectivity following treatment in patients with annoying tinnitus. researches showed that real and long-term 1 Hz rTMS treatment was capable of significantly reducing the tinnitus severity even in a group of patients with long-term symptoms resistant to pharmacological treatment.Conclusion: as the history of tinnitus is short patients respond better to rTMS . studies showed the significant correlation between change in handicap and change in depression. Some studies believe that laterality of stimulation is not important and some of them believe contralateral stimulation have a greater beneficial effect on symptoms than either ipsilateral or left side stimulation
