THE ASSOCIATION BETWEEN FOOD SECURITY AND DEPRESSION IN FREE-LIVING OLDER ADULTS OF TEHRAN CITY

Background and Aim: To determine the relationship between food security and depression among urban free-living elders in Tehran city Methods: This cross-sectional study was carried out in 505 free-living elderly aged 60 to 80 in Tehran city. The subjects were chosen by a systematic multi-stage cluster sampling from all the city geographical zones. Food security was measured by two questionnaires (adult part of US-Household Food Security Survey Module[US-HFSSM] and 8-item Direct Experience-Based Elderly Food Insecurity Measurement Tool [DEBEFIM]) that were previously validated for Iranian elders. Depression status was measured by a validated 11-item geriatric depression scale. Data were analyzed by SPSS. Association of food security status and depression was assessed by Logistic Regression. A P<0.05 was considered as significant.Results: Mean (SD) age of participants (242(47.9%)) women and 263(52.1%) men) was 67(0.25) years. Based on the two scales (DEBEFIM vs. HFSSM) 71.6% vs.57.5% of elders were food secure, 16.1%vs.23.3% were food insecure without hunger, 9% vs.15.3% were food insecure with moderate hunger and 2.6%vs.3.8% were food insecure with severe hunger. After adjusting for confounders, those at the highest food severity insecurity status in comparison with food secure participants had a higher depression chance. The depression odds increased when the score of food security decreased based on both US-HFSSM(OR=2.18, 95%CI: 1.6-2.9) and DEBEFIM(OR=2.43, 95%CI: 1.7-3.3) [p-trend <0.01].Conclusion: Comparing the prevalence of food insecurity among Tehran elders with international reports reveals a higher prevalence. Findings suggest that depression is associated with elders food security status. However, cohort studies are needed to determine the causality of this association.
