Molecular Detection of Vancomycin andGentamicin Resistance Genes amongClinical Vancomycin-Resistant Enterococcusfaecalis and Enterococcus faecium Isolates

Background: The presence of vancomycin-resistant enterococci (VRE) imposes many problems for healthcare systems such as an increase of the length of stay and financial burden, therefore, it is vital to monitor the prevalence and investigate the involved resistance mechanisms. The aim of this study was todetection of vancomycinand gentamicin resistance genes among clinical vancomycin-resistant Enterococcusfaecalis and Enterococcus faeciumisolatesin the Northwest of Iran. Methods: Two vancomycinresistance-associated genes, vanAand vanB and gentamicin resistance-associated gene, aac (6′) Ie-aph (2″) Iawere detected by the multiplex PCR, using specific primersin 30 vancomycin-resistant E. faecalisand E. faeciumstrains isolated from various clinical specimens. Results: Among the 30 VRE isolates characterized in this study, 27(90%) carried the vanA gene, while vanB gene was not detected in any of them. All high-level gentamicin resistant (HLGR) isolates (n= 19; 63.33%) were positive for aac (6′) Ie-aph (2″) Iagene. Conclusion: The commonest genotype of glycopeptides resistance distinguished in our study was the vanA genotype. These results highlight the relevance of VRE monitoring constantly in these and other setting to detect future possible changes in the prevalence and genotypes of this important multidrug-resistant pathogen, which holds special significance in human health.
