Endogenous Endophthalmitis after Lower Limb Amputations of a Patient with Bürger Thromboangiitis Obliterans

Purpose: Endogenous endophthalmitis (EE) is a rare condition that is caused by hematogenous spread of bacteria or fungi and is usually seen in patients with predisposed medical conditions. We are reporting a rare case of EE after lower limb amputations (LLA) and infection of the stump in a patient with Bürger thromboangiitis obliterans (BTO).Methods: A 66-year-old man referred to us with chills, fever, left eye pain, and visual loss. His medical history revealed BTO and LLA two weeks ago. Owing to the acute onset of pain and infection of the stump of amputated limb and patient s bandemia, our suspicion was EE. Results: Pars plana vitrectomy and intravitreal injection of AB was performed. Blood cultures, lower limb stump cultures, and vitreous cultures grew streptococcus. The patient s vision was poor LP on discharge from the hospital.Conclusion: Amputation is the last treatment option in limbs, which are not salvageable or when the diseased limb poses a threat to the life of the patient with BTO. Infection of stump was the most common local complication in the post-operative period. EE provides a difficult diagnostic and therapeutic challenge. However, even with prompt treatment, visual outcomes may be poor.
